Commercial/Industrial Electric Service Construction Request Form (PDF) – Please return this form completed and signed to SUB’s Electric Service Center.

Residential Electric Service Construction Request Form (PDF) – Please return this form completed and signed to SUB’s Electric Service Center.

  1. Trenching Details ESD0074 (4-pages) (PDF): Includes backfill, trench dimension, and street crossing requirements.
  2. Typical Joint Trench Detail #FF11DE64 (1-page) (PDF): Guide for trench proximity to property lines and existing or planned utility/communication facilities as well as the orientation of utility/communication conduits within a joint trench.
  3. Secondary Junction Box Entrance ESD0075 (1-page) (PDF): Guide for the installation of secondary junction boxes and orientation of conduits entering them.
  4. Vault Installation Notes and Details ESD0070 (4-pages) (PDF): Concrete vault installation requirements including backfill, grounding, and proximity requirements. Conduit orientation when entering a vault and spacing requirements also addressed.

  18. ESD0118 – Multi-Story Metered Installations:This standard specifies metering requirements for high-rise, multi-story residential, commercial, and mixed-use buildings that include rental and/or owner-occupied dwelling units and/or commercial units.
  19. ESD0118-A – Installation Checklist: This checklist will be exchanged between SUB and the Customer multiple times to advance past hold points as described in ESD0118. SUB written approval will be required for each hold point.
