COMMERCIAL/INDUSTRIAL ELECTRIC SERVICE CONSTRUCTION REQUEST FORM – Please return this form completed and signed to SUB’s Electric Service Center.

1. Trenching Details ESD0074 (4-pages): Includes backfill, trench dimension, and street crossing requirements.

2. Typical Joint Trench Detail #FF11DE64 (1-page): Guide for trench proximity to property lines and existing or planned utility/communication facilities as well as the orientation of utility/communication conduits within a joint trench.

3. Secondary Junction Box Entrance ESD0075 (1-page): Guide for the installation of secondary junction boxes and orientation of conduits entering them.

4. Vault Installation Notes and Details ESD0070 (4-pages): Concrete vault installation requirements including backfill, grounding, and proximity requirements. Conduit orientation when entering a vault and spacing requirements also addressed.

5. Typical Overhead Temporary Service ESD0100 (2-pages): Includes drawings with spacing/clearance requirements for the installation of temporary overhead services.

6. Typical Overhead Mobile Home Service ESD0103 (2-pages): Includes drawings with spacing/clearance requirements for installation of overhead service to a mobile home.

10. Typical Underground Temporary Service ESD0104 (2-pages): Includes drawings with spacing/clearance requirements for installation of temporary underground services.

11. Typical Underground Service ESD0105 (2-pages): Includes drawings with spacing/clearance requirements and customer responsibilities for installation of underground services from a padmount transformer vault or junction box.

12. Typical Underground Service Feed Overhead ESD0106 (2-pages): Includes drawings with spacing/clearance requirements and customer responsibilities for installation of underground services that are fed from an overhead structure.

13. Temporary Mid-span Supports ESD0101 (2-pages): Includes drawings with spacing/clearance requirements for the construction of temporary structures to support overhead service conductors. 

14. Customer Requirements for Meters ESD0090 (7-pages): Includes meter installations requirements.

15. Developer Supplied Materials (1-page): Includes SUB specifications for conduit and mule tape that may be required for a development project.
